REFER A CLIENT

Are you a Social Worker, hospital-based Discharge Planner and/or someone looking to refer a loved one to quality, supportive, home care services?  If so, you're in the right place.  At ASI, we welcome client referrals and are happy to make the process of referring a client as straightforward as possible.

As a referrer, you can expect: 

• Professional ASI staff members available by phone to answer any questions you may have;  

• Multilingual/ Multicultural service offerings and client communications;

• Professional service providers, licensed and certified to provide home care services in Illinois by the Illinois Department on Aging;

• Quick intake process, with caregiver/ client matching typically occurring within 48 hours of the intake process being initiated

Once we've received your information, an ASI intake specialist will contact you for a brief follow-up call to discuss the specific service needs of the client and schedule an on-site visit with the client.  On the follow-up call we will work with you to determine if ASI can accommodate your specific client's needs; estimate the hourly commitment per service need; and outline the anticipated costs.
