​In partnership with the Department of Family & Support Services, the Aging Well Navigator program seeks to address the diverse needs of older adults, by meeting them where they are and bringing informational materials and resources to them as they seek to age well and with dignity.

How does it work?
Program Eligibility


Meet and identify the client’s need for assistance

Age 60 and over
-
Must Live in a Senior Building
Assist client with benefits check-up and overall resource assessment​

Interview the client and discuss their service needs​

Refer clients to options counseling and/or intensive case management support​
